We are NOT an emergency medical facility. Call 911 or seek emergency care if experiencing any life-threatening situations such as:
Anaphylaxis or severe allergic reaction, Cuts or lacerations with severe bleeding, Heart attack-like symptoms, Loss of consciousness, Overdose or poisoning, Active seizures, Severe dehydration, or severe shortness of breath or breathing difficulties, Severe pain, or severe vomiting or vomiting blood, Stroke-like symptoms (sudden weakness, speech or vision changes)

To help us submit your insurance claims accurately and maintain our quality of care, please provide the most up-to-date insurance information.
Important Insurance Information:
- While we accept many insurance plans, coverage is not guaranteed. Please verify with your insurance carrier that we are in-network.
- We are not in-network with HealthPartners or Cigna.
- Patients are responsible for confirming their benefits and for any costs not covered by insurance (including co-pays, deductibles, and co-insurance).
- Co-pays are due at the time of service. Failure to collect these is considered insurance fraud.
- We require a current copy of your insurance card. If you cannot verify active insurance, full payment is due at the time of your appointment. You may be refunded once insurance processes your claim.
Private pay, out-of-network, and uninsured patients must also pay in full at the time of service. We offer reasonable uninsured rates
Billing & Payments:
- If you receive a statement from LifeWay Health & Wellness, the balance must be paid before your next visit or by the due date—whichever comes first.
- Accounts over 90 days past due will be sent to collections. No further appointments will be scheduled until the balance is paid in full.
Lab Services:
We use Quest Diagnostics for lab testing. You may receive a separate bill from them. For billing questions, contact Quest directly at 1-866-MY-QUEST (1-866-697-8378).

Direct Primary Care Contract
This is for clients without insurance coverage, or whose insurance is not in network with us. It is a monthly fee, concierge service. Providing patients unlimited visits, direct access to their provider, low prices for labs, and procedures.
